We work closely with health insurance plans, Medicare, CenCal (MediCal) to maximize the portion that insurance covers. Serenity House room and board is not generally covered by Medicare or insurance. If qualified, subsidized care is offered on a sliding fee scale. Our charitable mission ensures that those who cannot afford are not turned away. The last rating for this service was requires improvement (published 22 January 2020). The service remains rated requires improvement. This service has been rated requires improvement for the last three consecutive inspections. On the day of our inspection staff interacted well with people in the residential home. People and their relatives using both parts of the service told us that they felt safe and well cared for. Staff knew how to keep people safe. The provider had systems and processes in place to keep people safe.

People were supported and encouraged to make choices and had access to a range of activities. Staff knew what was important to people and encouraged them to be as independent as possible. Staff sought consent from people before providing care or support. By getting help at our residential detox center, you’ll see that you don’t have to do this alone. While in treatment, you’ll see that there are many other people dealing with similar issues, and they’ll be there as part of your support system, along with staff and counselors. This will allow you to see that you’re not alone, and you never have to be alone again. Asked how they would rate the service, one person said they 'would give it 300%'. They added 'what more could I want? I've got a good landlady and a good staff team'. Both people using the service told us that they were 'loving it here'.Medicines were kept safely and appropriate arrangements were in place in relation to the recording of medicine. The person we spoke with confirmed that staff had the right knowledge and skills to support them. They told us how they were involved in the recruitment and selection of staff and that agency staff were not used in the home, in accordance with their needs and wishes. We saw that staffing levels reflected the assessed needs of people using the service, as identified in their support plans.

For adult social care services, the maximum time for being in special measures will usually be no more than 12 months. If the service has demonstrated improvements when we inspect it. And it is no longer rated as inadequate for any of the five key questions it will no longer be in special measures. We observed that staff treated people with dignity and respect. There was a relaxed atmosphere in the home and staff communicated well with the people who used the service and promoted an inclusive, supportive environment. Staff had a good understanding of people’s needs and were provided with training on a variety of subjects to ensure that they had the skills to meet people’s needs. The provider had a training plan in place and staff had received regular supervision. Staffing levels were not always consistent. This meant people did not receive a person-centred approach which included the one to one support they needed. Systems and processes to protect people from the abuse and improper treatment was not robust. People were at risk of harm because risk assessments were not always place for specific risks to people. System to monitor and review risks and support plans was not effective. People were not always involved in the process to assess, develop and review risks and their support plans.The service also provides personal care to 50 people living in their own homes in Gainsborough and in Grantham. People's needs had been assessed before they moved into the home and each person had a detailed care plan, which showed that they had been involved in agreeing how staff would support them. We saw that the service liaised with other health and social care professionals, which helped to ensure that the delivery of care was responsive to people's needs and based on up to date information and guidance. The records showed that any concerns were followed up and appropriate action was taken.

Our Serenity House team includes physicians, all of whom are board certified in hospice & palliative medicine, nurse practitioners, registered & licensed vocational nurses, home health aides, medical social workers, bereavement counselors, chaplains, pet therapists, a certified music therapist, a registered dietitian, a chef, and specially trained volunteers. Anyone who spots something suspicious can always report it to our Customer Service team so that our fraud team can investigate. People told us they felt safe with the staff who supported them. Staff we spoke with had good knowledge of their responsibilities in relation to protecting people from abuse. Information in people’s care plans gave staff the guidance to provide effective support. People’s medicines were managed safely, and staff showed a good understanding of how to reduce the risk of infection for the people they cared for. Integrative Therapies include soothing practices such as guided imagery, gentle reflexology, mindfulness practice, reiki and aromatherapy. These therapies increase comfort and provide a sense of well-being. We are committed to being transparent about our costs. Once someone has paid for their treatment there are no additional charges or hidden costs for any of the services we provide as part of a client’s core treatment while at Serenity House. North Warren provides care for older people who have mental and physical health needs including people living with dementia. It provides accommodation for up to 15 people who require personal and nursing care. At the time of our inspection there were 13 people living at the home. The provider also provides personal care to people in their own homes. At the time of our inspection the provider was providing personal care to 42 people. Rating at last inspection: Requires Improvement (Report Published). At our previous comprehensive inspection in October 2017 the service was rated overall requires improvement. We found a breach of Regulation 17 HSCA 2008 (Regulated Activities) Regulations 2010. The service had been developed in line with the values that underpin the Registering the Right Support and other best practice guidance. These values include choice, promotion of independence and inclusion. People with a learning disability were supported to live as ordinary a life as any citizen.
