The CPS paper is up next and has 97 clinical questions in 75-minutes of which 86 are scored and 11 are being piloted. The CPS paper tests five main competencies: investigation, diagnosis, emergency, prescribing and management (non-prescribing). 1 The questions are posed in straightforward language and do not test knowledge that is specific to certain areas to ensure fairness for all candidates. The Multi-Specialty Recruitment Assessment (MSRA) is a single exam assessed using a computer-based system. It is to be taken by individuals in their recruitment year who are applying into the following postgraduate medical training: General Practice, Surgery, Psychiatry, Radiology, Ophthalmology, Obstetrics and Gynaecology, Community and Sexual Reproductive Health, and Neurosurgery. The MSRA scores awarded for GP applicants in each paper are normalised around a mean score of 250 with a standard deviation of 40. In addition, the scores for each paper are banded 1 to 4 where 1 indicates that the minimum acceptable standard has not been achieved. Applicants who achieve a band of 1 in either paper will be deemed unsuccessful and will not progress further with their application. When Should You Start Preparing For The MSRA?

The MSRA application for 2024 Round 1 opens at 10am on 26 October 2023 and closes on 23rd November 2023 at 4pm. The 2024 MSRA resource is authored by practising UK doctors who have successfully completed training posts in the past. We know the importance of relevance, accuracy and clarity when spending the limited time you have revising. Section one of PD paper uses ranking questions. You get a scenario, there's four or five options and you rank them best to worse. In section two of the PD paper there's a scenario, there's eight options and you're asked to select the three most appropriate options from the list. The Professional Dilemma Paper – or SJT – is a 95 minute exam paper consisting of 50 questions. It is usually taken first and then followed by the second part of the MSRA test – The Clinical Paper. It assesses 3 core competencies: The MSRA is delivered in Pearson VUE test centres across several consecutive days within the MSRA window. The exam is offered throughout the UK and internationally; you can find your closest test centre here . It’s worth noting that there may not be availability at your nearest or preferred Pearson VUE test centre, due to the demand within the short assessment window, so you may be required to travel further to sit your exam.It does not test knowledge or problem-solving. Instead, the content relates to the GMC’s Generic Professional Capabilities framework and tests attributes that are important for doctors’ training progression and job performance. The three main competencies tested are professional integrity, coping with pressure and empathy and sensitivity. 1 Knowledge of specialty training is not needed but a general understanding of primary and secondary care work is expected. Questions are set in the context of the UK Foundation Programme; candidates assume the role of F2 doctors. However, to make certain the fairness of the MSRA for all candidates including International Medical Graduates, questions do not entail any knowledge of policies or procedures that are specific to the UK. 1

For both the PD and CPS sections the candidate will be given a standardised score that comes under a specific band (1-4). For example, those who score <180 in each exam are limited to Band 1, which represents a ‘very poor level of performance’. Scoring over 291 in the exam puts the candidate in Band 4, illustrating a ‘very good level of performance’ in the exam. Additionally, 181-230 is Band 2, representing a ‘below average performance’ and 231-290 puts the candidate in Band 3 or a ‘good level of performance’ in the exam. What specialties need MSRA? Half of the questions in the PD paper are ranking questions and half are multiple-choice questions. Typically, the answer options are actions that can be taken in response to the circumstances in the scenario. Good, acceptable, and poor actions are included as options, as is the best action. Unrealistic actions are not included. The MSRA exam was first introduced in 2013 as part of the selection process for core medical training, and has since been expanded to cover a wide range of specialties.

What is the Format of The MSRA?

A is a poor option as it potentially breaches patient confidentiality and may cause distress to the patient and relative. You are taking action to try and ensure patient understanding but going about it in a way that i) undermines the registrar, ii) may negatively impact your working relationship and iii) may negatively impact the patient and family’s trust in the medical care being provided.

The Clinical Problem Solving paper is timed, and candidates have 75 minutes to complete it. Although there is no negative marking, candidates are advised to answer all questions to maximize their chances of obtaining a high score. After candidates have filled in their application for GP specialty training, and the deadline for application submission has passed, they are provided details to register with Pearson VUE. Applications are assessed using the criteria listed in the General Practice – ST1 Person Specification. Subsequently, longlisted candidates are invited to sit the MSRA. Candidates must book a test for their preferred centre, date, and time by logging onto their Pearson VUE account. The MSRA was originally introduced as part of specialty selection for GP training and has gradually been rolled out to most other specialties. It's basically used as a way to shortlist candidates before selection interviews and it features a mix of clinical multiple choice questions like finals and situational judgement questions like the SJT split across two papers. In the CPS part of the assessment, candidates are presented with clinical scenarios that require them to exercise judgement and problem solving skills to determine appropriate diagnosis and management of patients. Questions focus on your ability to apply and apply clinical knowledge appropriately. The items are based on Foundation-level clinical practice, and test higher level knowledge of medical knowledge. You are allowed to take the MSRA a maximum of three times, although this varies depending on the specialty. If you do not achieve a satisfactory score after three attempts, you may not be eligible to apply for the specialty in question. This is fine if you’re the kind of person who completes multiple question banks, multiple times before an exam. It doesn’t matter if the questions you answer aren’t targeted because by doing thousands and thousands of questions, you’ll eventually cover your knowledge gaps anyway. My simple answer is that relying on one method alone can leave you a little short. No question bank can cover every aspect of the MSRA, whilst purely reading books and guidelines will not give your brain adequate exposure to problem-solving. If planned well, both should be used together as both have advantages in terms of improving retention and performance. Investigation– Focusing on specific investigations to support a more conclusive diagnosis or to rule out possible causes of illness / injury. Please note: currently, all MSRA invitations will be for MSRAs delivered within a Pearson VUE test centre; however, once you receive your invitation, you can apply for a remote test ‘by exception’ if you meet the criteria set to do so. These build to form a reference library you can access at any time SBA questions present a clinical scenario and ask the candidate to select the single best answer from a list of options. Each SBA question usually requires the candidate to apply clinical knowledge and reasoning skills to identify the most appropriate diagnosis or management option for the given scenario. I signed up to the Pastest question bank 5 months before the January exam- I knew I would have to start preparing early with 2 children under 3. I slowly worked my way through the question bank, doing about 2-3 hours a week and making notes of and rehashing weak areas. We were still living in Malawi and our internet was terrible, so I managed to get hold of a few books of practice questions from Pastest and ISC Medical.
